First visit
We welcome new patients.
At The Dental Health Group we want to create a professional, friendly welcome that will be the start of a positive dental experience for you.
When you first walk in the door, you will be greeted at reception by one of our friendly team. You will be asked to complete a medical and patient questionnaire. If you do take a number of medications it is a good idea to bring your prescription form from the pharmacy with you.
Should your visit be ACC related there may be further paperwork to complete. |
